Job overviewThe Community Rehabilitation and Homefirst teams are part of the Penwith Integrated Community Team. The teams comprise of registered and non-registered nursing, therapy and administrative staff, working collaboratively with partner organisations within the Neighbourhood to support the intermediate and rehabilitation needs of our local population.
This position is open to physiotherapists with community experience who wish to develop assessment, care planning, reablement and rehabilitation skills whilst being part of a dynamic, research-active, progressive, integrated, multiagency team.
You'll be responsible for assessment, planning and implementation of care, and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs and set realistic rehabilitation goals. You need excellent organisational skills along with a wide knowledge base and experience of working in older person's services.
We offer a well supported working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system.
This post covers the Penwith are which spans from Hayle to Marazion and all areas to the West.
The service covers a 7 day rota which involves some weekend and bank holiday working: the successful candidates must be flexible with their working days. We cover 8am-6pm

Main duties of the jobYou will need to be able to triage referrals to identify need and urgency, assess and provide highly specialist evidence based treatment plans for adults within the catchment area, either in their own homes or care homes.
To work alongside the wider multidisciplinary teams within the community which includes partner agencies, liaising with the community and acute hospitals, Adult Social Care and the Voluntary Services

Person specificationeducation/qualifications and relevant experience
Essential criteria
- Degree or diploma in Occupational Therapy or Physiotherapy
- HCPC reigstered
- Substantial post registration clinical experience in a similar role
- Extensive experience of working within a multidisciplinary team
- Experience of managing the risks associated with working in a lone working environment
- Evidence of appropriate postgraduate courses and CPD to support practice, to degree equivalent level
- A broad range of clinical experience in a variety of community and inpatient settings
Essential criteria
- Confident and competent leadership with the ability to work in diverse and complex situations
- Ability to make clinical risk-management decisions in lone-working situations
- Awareness of safeguarding policies and procedures

- Highly developed verbal and written communication skills that will enable to post holder to work effectively across boundaries and in a variety of settings.

- Clear understanding of clinical effectiveness and the need to provide evidence based care
- IT literate
- Teaching, Assessing, coaching and mentoring skills to facilitate an effective learning environment for staff and students
