Role Overview

  • Crucial role in supporting vulnerable adults in the community
  • Work closely with individuals, families, and professionals.

  • Compassionate approach and strong assessment skills contribute to positive outcomes.

Key Responsibilities

  • Assessments and Support Planning
  • Conduct comprehensive assessments considering various factors.
  • Coordinate services with health professionals and community resources

Safeguarding and Risk Management

  • Promptly identify and respond to safeguarding concerns.
  • Assess risks and implement interventions.
  • Ensure safety and legal compliance.

Direct Work with Clients

  • Provide emotional support, advocacy, and access to services.
  • Facilitate group sessions to enhance social skills.

Collaboration and Multi-Agency Working

  • Liaise with healthcare professionals, police, etc.
  • Advocate for clients' rights in multidisciplinary meetings.

Documentation and Record Keeping

  • Maintain accurate case records and ensure compliance.

Qualifications and Experience

  • Degree in Social Work or equivalent.
  • Registration with Social Work England.
  • Experience in adult social care.
  • Knowledge of relevant legislation.
