As a PCN Clinical Pharmacist, you will act within your professional boundaries, supporting and working alongside a team of pharmacists in general practice. You'll work as part of a multi-disciplinary team in a patient-facing role, taking responsibility for chronic disease management and undertaking clinical medication reviews to proactively manage patients with complex polypharmacy. You'll also provide clinical leadership on medicines optimisation, quality improvement, and support practice staff with prescription and medication queries.
