To independently actively monitor the multidisciplinary management and
discharge plan for all patients on designated ward(s) and take action to expedite the process and thereby improve the patients' experience.
A critical success factor in this role is reducing the length of stay of all patients. For some patients this will mean reducing stay by hours, for others by days and weeks.
A pivotal part of the role will be the as a Trusted Assessor. A trusted Assessor will undertake a holistic assessment of needs that avoids duplication and speeds up response times from Care Homes enabling people to be discharged in a safe and timely way.
To support the delivery of a high quality, safe and compassionate healthcare service all staff are expected to act as a role model to others in all aspects of their work and consistently demonstrate QEHKL values of Kindness, Wellness and Fairness.
To have an up-to-date knowledge of the multidisciplinary management and discharge plans of all allocated patients, updating patients, and their
carers/families as appropriate in liaison with ward multi-disciplinary team (MDT).
If a clear plan cannot be identified in the medical record, to contact relevant clinicians for clarification. Expectation is to manage more complex discharge cases with the support of the Complex Discharge Sisters.
To independently gather and collate information from the medical notes,
patients and multi-disciplinary team colleagues to enable a clear plan for
discharge to be followed.
To undertake and support others in assessments which facilitate effective and timely discharges from acute hospital and community settings, ensuring people are discharged to the right setting with complete, accurate and up to date information and associated documentation. To maintain a strong, visible presence providing expertise and assessment skills in acute, community and care home settings.
To provide data, co-ordinate discharges and provide administrative support to the multi-disciplinary team within the hospital to improve joint working practices leading to more effective patient care and timely discharges.
