Job summary

Are you an experienced Mental Health Professional looking to utilise your clinical skills in a different and rewarding environment?

The Isle of Wight Dementia Outreach Team (DOT) are seeking an ambitious and caring Senior Registered Mental Health Practitioner to join our existing team.

The Dementia Outreach Team are proactive and highly skilled at managing risk and supporting service users on the Isle of Wight who are presenting in crisis. The DOT service are passionate about providing alternative options to hospital admissions, so you can expect to be undertaking rewarding and highly intensive work.

We looking to recruit a compassionate, dedicated, and conscientious individual, who wants to make a positive difference to adults experiencing a dementia-related crisis.

Staff provide care, treatment, and assessment to patients in their own homes and other places of safety, getting you out and about seeing patients in a range of community settings.

Main duties, tasks & skills required

To ensure and take responsibility for the provision of comprehensive nurse assessment to support the MDT to diagnose a dementia related illness and develop a plan of care post diagnosis of people referred to the memory assessment service.

To plan and take responsibility in collaboration with service users and their carers together with other services/agencies on how people's needs are going to be met in short-term and longer-term situations.

To be active in the development of the service and continued development of professional practice and role.

To ensure that relationships and own behaviour are managed in line with the Trust's Equalities Framework competencies as well as professional codes of practice and the organisations values and behaviours framework.

To ensure all communications are undertaken in a sensitive, confidential and professional manner.

Interdisciplinary working and liaison with internal and external agencies to promote holistic and person-centred, strength-based care and inclusion.

RMN or Allied health professional (Mental Health) - Evidence based and experience of reflective practice post registration.

Current national legislative and professional requirements relating to social and health care and processes implemented locally (The Care Act 2014, The Mental Capacity Act 2005, Mental Health Act 1983)

Mentorship /supervision qualifications/training
