Role Purpose

The Scheduler & Care Co-ordinator plays a central role in the safe, effective, and person-centred delivery of West Park Care's community Home Care Service; general, enhanced and complex.

The role is responsible for coordinating care delivery, supporting continuity, assisting with onboarding and workforce planning, and working alongside the Service Manager to support new client assessments and care planning where required.

Key Responsibilities

Scheduling & Care Coordination

  • Create, manage, and maintain accurate staff rotas and care schedules for community-based home care
  • Coordinate care delivery across the service to support continuity and consistency
  • Match staff to clients based on skills, training, competency, availability, and compatibility
  • Respond effectively to short-notice changes such as sickness, emergencies, or changes in care needs
  • Ensure safe staffing levels and appropriate cover at all times, including liaison with on-call staff

New Clients, Assessments & Care Planning

  • Support the Business Development Manager with onboarding new clients
  • Attend initial assessments in the community with the Business Development Manager
  • Contribute to care planning by supporting accurate scheduling, visit structure, and staffing allocation
  • Assist with setting up care packages in line with assessed needs, risk assessments, and care plans with support of the Business Development Manager.
  • Support timely implementation of new care packages following assessment

Induction Coordination & Workforce Planning

  • Support the coordination and planning of staff inductions with the Deputy Manager and Supervisors
  • Assist with scheduling induction activities, including shadow shifts and competency sign-offs
  • Ensure new starters are not scheduled independently until induction requirements are completed
  • Support safe deployment of staff by considering skills, experience, and induction progress

Communication & Working Relationships

  • Act as a point of contact for clients, families, and staff regarding visit times and coordination
  • Communicate changes clearly, promptly, and respectfully
  • Work closely with Managers, Supervisors, and the wider office team to ensure joined-up working
  • Escalate concerns, incidents, or safeguarding matters appropriately

Quality, Safety & Compliance

  • Ensure care is scheduled in line with care plans, risk assessments, and service policies
  • Maintain accurate and timely records within care management systems
  • Support compliance with CQC requirements, including safe staffing and continuity of care
  • Report and escalate risks, incidents, or concerns in line with policy

Exceptional Support

  • In exceptional circumstances, provide direct care support to maintain service continuity, where trained and competent to do so

Professional Responsibilities

  • Maintain confidentiality and data protection at all times
  • Work in line with West Park Care's values, policies, and procedures
  • Participate in training and development relevant to the role
  • Support on-call cover in line with service procedures – second line.
  • Undertake other reasonable duties as required

Person Specification

Essential

  • Experience scheduling home care services in the community
  • Strong organisational and communication skills
  • Ability to work calmly under pressure and manage changing priorities
  • Excellent interpersonal skills and building relationships
  • Confident IT skills and experience using care scheduling systems
  • Understanding of person-centred care and continuity

Desirable

  • Experience supporting care package setup, assessments, or care planning
  • Knowledge of CQC requirements
  • Level 3 Health & Social Care qualification (or willingness to work towards)

Pay: £28,000.00-£30,000.00 per year
