Palliative Care Staff Nurse - Access / Community Team / Virtual Ward
Hospice Band: 5
Starting Salary: £37,335 WTE (rising to Step 2 £41,424 WTE subject to 12 month satisfactory performance review) pro rata per annum + Pension + Benefits
Part time 22.5 Hours per week (weekdays)
An exciting opportunity has arisen for a caring and compassionate registered nurse to join our Hospice Team. You'll be a Registered Nurse with palliative care experience, excellent interpersonal and communication skills and a proven ability to work as part of a multi-professional team.
Our Palliative Care Nurses provide specialist advice, guidance and support to patients at home, in care homes and our Inpatient care. The Staff Nurse role involves getting to know the patient and their family well enough to allow them to talk about how they are feeling, their worries and anxieties. As well as advising on symptom control, medicines and treatments, our nurses ensure people are getting the practical help they need, from community partners.
You will be responsible for initial and ongoing holistic assessment of new patients and planning of care to meet the patients and carers practical, psychosocial, spiritual needs, as well as responding to queries requests for advice and support form patients, families and colleagues. This is a responsive/reactive role suited to experienced staff nurses with palliative care experience. This post will involve rotation between the Access team and community setting, although initially office based, a full driving license and use of your own car is required for patient home visits.
This is an extremely supportive team environment. This initial work is key to ensuring that everyone in need of hospice services receives support as quickly and comprehensively as possible – and is triaged to the most appropriate team for follow up.
