Your Quality Care Services are a well-established home care provider and we pride ourselves on delivering the highest quality care possible. We are looking for motivated and caring individuals who share our ethos and want to make a real difference in the work that they undertake. We are specifically looking for a care coordinator in our branch located within a retirement village setting in the Dunchurch area.

Salary of £28,000 is pro-rata.

Overall Purpose of the Job

  • To be responsible for the effective rostering of staff resources to ensure contractual requirements are met and that the provision of care for clients is within companies the policies and procedures and meet National Care Standards and CQC requirements.
  • To be responsible for ensuring that all care visits are delivered including arranging covering staff during periods of annual leave and sick leave.
  • To carry out assessments of clients individual needs and complete risk assessments of the environment and activities that are to be carried out during the provision of care.
  • To develop partnerships and good working relations with statutory and voluntary agencies in order to achieve the delivery of high quality care that ensures that individual client needs are fully met.
  • To be responsible for reviewing all visit log records and inform the registered manager of any issues or changes in clients individual needs.

Roles and Responsibilities

  • Create new client records on the scheduling system ensuring that all information recorded is kept fully up to date and is accurate.
  • Arrange and carry out client needs and risk assessments within 48 hours of referral (subject to agreed time with the client) and prior to introduction of care staff.
  • Write detailed, informative and accurate care plans that are to be maintained within the client's home and the office records. Ensuring that required consents have been given by the client or the responsible person and care plans are fully signed.
  • Allocate suitably matched staff to the client in order that the clients care needs are fully met.
  • Ensure that the most effective and efficient allocation of staff is scheduled in order to meet required visit times, transportation requirements and staff availability.
  • Monitor the workload of new and existing care support staff and ensure that required hours are being allocated.
  • Ensure that the registered manager is informed of potential increased staffing requirements in order that recruitment can be carried and to ensure that new referrals are able to be appropriately staffed whilst maintain current care delivery requirements.
  • Carry out reassessments of client care packages as per the individual clients support needs (minimum of annual reassessment) and ensure that care plans are appropriately updated with any changes required.
  • Ensure that any unallocated visits due to both planned and unplanned staff leave are covered, ensuring that the scheduling system is updated and that staff and clients are informed of any changes made.
  • Co-ordinate delivery of care whilst ensuring that all staff attend staff meetings, supervisions and training within CQC required timescales.
  • Carry out formal supervision, direct observations and spot checks of care staff on a minimum quarterly basis. Ensuring that all details are appropriately recorded and filed.
  • Have a working knowledge of the companies policies and procedures and ensure that care staff are delivering the service as per the requirements of the policies and procedures.
  • Attend and (in the event that the registered manager is unavailable) facilitate quarterly staff meetings.
  • Participate in the out of hours on call duty rota.

OTHER

  • Attend training and study days etc as agreed in the personal training and development plan, developed in consultation with the Registered Manager.
  • To attend a minimum of quarterly supervisions and an annual appraisal with the Registered Manager.
  • Carry out additional duties or tasks related to ensuring delivery of high quality care services

Job Type: Full-time

Pay: £28,000.00 per year
