An exciting opportunity has arisen to join our
Personalised Care Team
. Personalised Care gives people the same choice and control over their mental and physical health that they have come to expect in every other aspect of their life. Personalised Care is a key element of the NHS long term plan. It helps a range of people, from those with long term illness and complex needs through to people managing mental health issues or struggling with social issues which affect their health and wellbeing. It helps them make decisions about managing their health so they can live the life they want to live based on what matters to them, working alongside clinical information from the professionals who support them.

Social Prescribing works with people in a holistic way to help empower them to take better control of their health and well-being. It provides people with the time to talk about 'what matters to them', connecting them to community-based activity and statutory services that provide practical and emotional support.

You will work with a wide range of people, their families, and carers to help them to take control of their wellbeing, live independently and increase involvement with local communities. You will be required to build trusting relationships, using an asset and strength-based approach and provide personalised support to help people and their families to live well.

You will predominantly be working in North Kirklees. However, there will be an expectation that you are able to work across Kirklees as required by the needs of the service.

Willingness to work flexible hours, including evenings and weekends on occasion.

This is a 12 month fixed term contract.

The interviews for this role are expected to take place W/C 1st December.

Jo Bolland is the manager for this role, please contact them on for an informal discussion, or if you need any more information.
This job is subject to a DBS check at the appropriate level, please refer to the Job Description/Profile. A conviction may not exclude candidates from appointment but will be considered as part of the recruitment process.

We know there's a wealth of talent among people who have a disability and we encourage applications from people with all differing abilities. So, if you need any support completing an application form, or any other format for the application or Job Description please contact the Recruitment Team for help by email:or phone: and ask for 'Recruitment'.

Applications for secondments will only be accepted if approved by your manager, you will be asked to confirm on your application form that you have the relevant permission. If you are refused permission to apply for a secondment, please contact HR Recruitment to register the refusal.

We are committed to safeguarding and promoting the welfare of vulnerable adults and children and young people and expect all staff and volunteers to share this commitment.
