Join our innovative Learning Disability, Transition and Autism Team, where no two days are the same and where your professional voice helps shape lives, services and outcomes. You will carry out person-centred assessments, plan strengths-based interventions and support people with a range of needs, including those that may be complex. Strong communication, effective risk planning and collaborative working are essential.
