Job Purpose To have continuing responsibility for the maintenance of a high standard of care to the patients and carers on the DN caseload To contribute to the continuing responsibility/management of the caseload, DN team and related resources To contribute to strategies and policies for DN service improvement and development To take an active role in the delivery of Public Health agendaDimensions To be cost conscious The case load size will be variable. To provide mentorship and leadership to clinical staff working within the specialty To work collaboratively and across organizational boundaries to provide evidence based practice and holistic care to patients in a variety of settings.Key Responsibilities To be responsible for the holistic assessment of the health and wellbeing needs of referred patients and carers, using analytical and interpretive techniques. Contribute to the Health Equity Needs Assessment of the local community. (Community Health Profile) Contribute to the Public Health agenda by raising awareness of health and wellbeing in the local community To have continuing responsibility supporting the District Nurse Team Leader with the planning, delivery and evaluation of programmes of care to address the complex and changing health and wellbeing needs of individuals and communities.
Utilise enhanced skills and knowledge to provide specialist technical services and advice in relation to care e.g. palliative care, leg ulcers, continence. Implement the provision of evidence based/best practice. Participate in public health and health promotion activities in response to identified local need.
Contribute to the involvement of the public in the planning and evaluation of the District Nursing Service Actively encourage patient and carer feedback about the DN service Act as a role model, setting, monitoring and evaluating the high standards and continual improvement to the DN service. Maintain a professional image as outlined in the NMC Code of Conduct Promote the Clinical Governance agenda and apply risk management and reporting systems Participate in relevant research and audit activity. Promote the provision of care by the team, which is consistent with evidence based/ best practice. Identify needs and prioritises work appropriately to manage the delivery of the service.
Take responsibility for the appropriate delegation and supervision of work according to the skills and abilities of team members. To be responsible for ensuring that the standard of record keeping by self and the team meets PCT and Professional standards. Establish and maintain communication with individuals and groups in a wide variety of settings and situations at all levels within the organisation and the wider health and social care community. Establish and maintain effective team working within the DN and Primary Health Care Team.
Motivate the team and implement change management processes. Set team objectives to continuously improve the standard of care and develop the service. To have continuing responsibility for performance monitoring of team members e.g. sickness and absence, competencies, complaints Regularly has responsibility for the management and co-ordination of the PCT wide DN service at weekends and bank holidays.
Contribute to the recruitment of staff through involvement with short-listing and interviews. To be responsible for the development of own and teams knowledge and clinical practice. To be responsible for the performance review and development interviews of the team in order to identify training and development needs. Participate in the supervision framework both as a supervisor and supervisee.
Maintain a portfolio to demonstrate continuing professional development. Mentor and assess a variety of students including nursing and allied professionals. To be actively involved in the continuous development of the DN service and policies procedures and guidelines. Represent the PCT DN service on multi-professional developments which support interprofessional working.
To be responsible for the promotion of best practice that ensures the health, safety and security of self, patients and colleagues in accordance with the policies procedures and guidelines of the PCT. Practise in accordance with procedures and guidelines for the Protection of Vulnerable Adults and Child Protection. Ensure that confidentiality is protected at all times.
