Due to demand, and continued success we require a case manager to expand our team.
What is Case Management?
Case Management is a collaborative process which: assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individual's health, social care, educational and employment needs, using communication and available resources to promote quality, cost and effective outcomes.
About Us?
Amber Case Management Limited was established in 2011 with the aim and objective of providing high quality community based rehabilitation for people following a traumatic or acquired brain injury or catastrophic injury across the North West of England.
About You
You must have a registered clinical qualification and have around 5 years experience in clinical case managing.
A minimum of 5 years post qualification experience preferably in the field of acquired brain injury, spinal injury, neuro or orthopedic.
Evidence of community based work would be desirable.
A full clean driving license and access to a car is essential.
Full support, training and mentoring will be provided to the successful applicant. Following successful completion of your probation you will also be enrolled into our company life cover and sickness benefit scheme.
Working in adherence to CQC standards and solid experience in this field of expertise, we would like to meet with you, with the ideal outcome of offering you a full time position.
Must have a strong working knowledge of documentation skills, analysing information, decision making, research skills, verbal communication, written communication, people skills, resolving conflict, people management, integrity.
