An exciting opportunity has arisen with STH NHS Foundation Trust for a physiotherapist who is keen to specialise in care and rehabilitation of the elderly.
As a specialist physiotherapist you will be working alongside a team of other professionals within an MDT to deliver rehabilitation and ongoing care. The team provides therapy and nursing input for 98 beds that are based within 3 care homes throughout the City. As part of this role, you will be required to work flexibly across these units.
Our caseload is referred to us from the acute hospital wards for patients with complex conditions including fractures, frailty, cardiac and respiratory conditions, amputations and neurological conditions. Our patients are with us for a short-term rehabilitation programme to facilitate returning home safely and promptly.
You will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritise tasks and manage time effectively, and be confident in your decision making. You must possess appropriate clinical skills and reasoning to deliver safe and effective physiotherapy treatment to our elderly client group.
This post will also provide opportunities to supervise and develop other staff members/students. You must be willing to show commitment to continuous development as we are a big advocate for staff development. For example, we have supported staff to complete courses such as PD Warrior, Bobath Module, Moving and Handling Key Trainers, etc.
To carry a variable clinical caseload of patients within the Intermediate Care and Rehabilitation Beds Service (ICRB).
To assess, diagnose, plan and develop specialist physiotherapy intervention programmes for patients with a wide variety of complex physical and mental health needs.
To participate and lead on the planning, delivery and evaluation of ICRB services within a designated area/unit, holding responsibility for defined projects.
To be an active member of the interdisciplinary Intermediate Care team in the provision of patient-centred care.
To seek and get support from the Physiotherapy Clinical Lead as needed.
To supervise/train junior physiotherapists/therapy assistants/students working within the service.
To develop and provide clinical expertise in a designated specialism and participate in the dissemination of evidence-based practice across the service.
