Halton Borough Council is seeking a committed and compassionate Social Worker to join our Intermediate & Frailty Team. This is an exciting opportunity to support some of our most vulnerable residents, helping them maintain independence, improve well-being, and safely manage complex health and social care needs.

About the Role

As a Social Worker within the Intermediate & Frailty Team, you will:

  • Work closely with older adults and individuals with frailty and complex needs.
  • Carry out strengths-based assessments and develop effective care and support plans.
  • Collaborate with multi-disciplinary partners including health professionals, hospital teams, community services, and voluntary organisations.
  • Support safe hospital discharges and prevent avoidable admissions.
  • Undertake Mental Capacity Act assessments and safeguarding enquiries where required.
  • Promote independence, resilience, and high-quality outcomes for adults and their families.

What We're Looking For

We are seeking a practitioner who is:

  • A qualified Social Worker with SWE registration.
  • Experienced within adult social care, ideally with knowledge of frailty, hospital discharge, or intermediate care pathways.
  • Confident in risk assessments, safeguarding practices, and person-centred planning.
  • Skilled at working in fast-paced, integrated environments.
  • Passionate about delivering excellent care and improving the lives of Halton residents.

Job Types: Full-time, Temporary

Pay: From £35.00 per hour

Expected hours: 37 per week

Work Location: Hybrid remote in Runcorn WA7 1JF
