Transfer of Care Hub Coordinator

Our Transfer of Care Hub is a dynamic, multidisciplinary team that supports safe, timely and person centred discharges from hospital. We work collaboratively with acute, community social care and voluntary sector partners to ensure that patients received the right care, in the right place at the right time.
Our focus is on reducing delays, improving patient flow and delivering high quality Discharge to Assess (D2A) and Home First pathways. We are committed to delivering excellent standards of communication, proactive problem solving and strong partnership working to improve outcomes for the people we serve.
This post is based at Warrington General Hospital, reporting the the operational team manager.
The roleThe Transfer of Care Hub Coordinator plays a key role in supporting safe, effective and timely discharges from the acute hospital into the community. While the team provides a 7-day service, this post is Monday to Friday, 08:00 - 16:00 (flexible working will be considered) 37.0 hours per week.
As the Hub coordinator you will have a responsibility to contribute to the development of the Transfer of Care Hub and agreed ways of working. You will also be responsible for daily management of colleagues within the hub, and oversight of operational working. This will include guidance, advice, aims and responsibilities of colleagues' roles and responsibilities within the Hub.
An important factor to the successful management of the Hub is developing and maintaining partnerships within the team, Warrington and Halton Hospital Trust, Bridgewater Community Healthcare Foundation Trust, wider intermediate tier of services and working with service users, carers and families, and other statutory and voluntary agencies to ensure the best possible service to the residents of Warrington.
