Main Duties and Responsibilities:

To provide leadership to delivering quality improvement in relation to safety, patient experience and clinical effectiveness, embedding the ethos of a safety culture. To provide oversight to the Community Hospital Inpatient Units and the South Place across all teams to include leading innovation and learning from incidents.

To ensure a responsive and proactive approach to managing and learning from concerns and complaints.

Contribute to the management of the clinical audit process and review of internal incidents.

Working with the place to ensure a robust risk management system with appropriate escalation.

Leading the continuous improvement required for harm free care, safeguarding and infection control, monitoring practice and compliance.

Creating and promoting a learning culture supporting a change to practice in light of this.

To work with the PCN and other partners to deliver high quality care

Ensure internal assurance is adequate regards quality standards to ensure an accountable process including reporting to the CCG.

To explore current practice through quality improvement, identifying areas for development and working with staff in planning the appropriate change using research and experience to evaluate the outcomes.

To coordinate practice, procedures / systems for clinical areas

To provide professional leadership providing a strong clinical presence and developing clinicians and clinical practice, assuring the provision of a safe, effective and evidence-based service to patients.

Work in conjunction with Social Care Leads to ensure there is an agreed joint approach to establish integrated pathways to support evidence base and best practice.

Active participation in the Local Safeguarding Adults Partnership to support multiagency approach to Safeguarding across the Place, linking to wider Norfolk strategies. To be responsible for developing safeguarding strategies and procedures in conjunction with the safeguarding lead nurse to implement all aspects of the safeguarding strategies for the place.

Clinical:

To maintain clinical credibility by acting regularly as a clinical practitioner in own area of expertise, role modelling and supporting the development of staff and learners by working regular clinical shifts as part of the establishment.

To be highly visible, accessible and approachable to staff, patients and the public to ensure that open and honest communication channels are created and sustained.

To maintain an overview of the standards of care being delivered across defined clinical areas and instigate remedial action if necessary. This will be done in conjunction with the Head of PCN to ensure actions required are embedded across Health and Social Care.

To lead and ensure high standards of practice in line with standards of care and other Government targets are embedded in practice

To work with the ward / department and sisters / charge nurses / team leaders in assessing the clinical skills required delivering a safe and high quality service, identifying any deficits in this and developing programmes to address these issues.

To provide advice as required, both internally and externally

To act as a coach / mentor for members of staff

To maintain a high clinical profile through embedding processes for assuring quality

To lead on infection control initiatives, ensuring that all staff within the organisation comply with Trust and NHS policies.

Work with Ward Manager to ensure maintenance of the standards of PLACE and resolve all actions / recommendations from PLACE visits are dealt with appropriately.

To support the implementation of audit, NICE guidance and relevant standards, ensuring that a multi-disciplinary approach is taken to raise standards.

To ensure key practices are embedded to support high quality care for fragility patients, including dementia and end of life.

Ensure NEWS2 and RESPECT are adopted and staff are supported to deliver to a high standard.

Take opportunities to support the long term plan's vision for preventative care, making every contact count, adopting a coaching approach to empower patients to manager their own care.

Management:

Represent the Place at multiagency meetings (e.g.: clinical quality review group).

To ensure staff have individual performance reviews and personal development plans to ensure staff development and standards are achieved which facilitates the achievement of required competency, knowledge and performance.

To assist in the resolution of any conflicts within the clinical area, resolving informal complaints locally and assisting in the investigation of and response to formal complaints and QIRs as required.

To investigate accidents and incidents; identify and implement corrective action where necessary, in conjunction with the Clinical Quality Director and Operations Director.

Identify multiagency incidents and working with Head of PCN to ensure shared learning across Health and Social Care.

To ensure there is an effective and efficient use of resources within the clinical areas and will be an authorised signatory.

To ensure that patients follow agreed pathways of care and are supported towards safe and effective discharge.

To be responsible for ensuring the Health and Safety measures are implemented and adhered to.
