On Call Care Coordinator

Job Summary
We are seeking a compassionate and dedicated Care Coordinator to join our team. The ideal candidate will play a vital role in ensuring the highest quality of care for our clients, particularly in assisted living, supported accommodation and home care settings. This position requires a strong understanding of patient care, including experience with Alzheimer's and dementia care, as well as a commitment to supporting seniors and their families through various stages of care.
Duties
- Coordinate and oversee patient care plans to ensure all needs are met effectively.
- Collaborate with healthcare professionals, families, and caregivers to facilitate communication and support.
- Assist carers for any queries in the administration of medication and monitor patient compliance.
- Maintain accurate adjustments of care calls on the live roster system and ensure staff attends their care calls.
- Ensure staff are not late for their care calls by monitoring the live roster care call monitoring system.
- Maintain accurate records of patient progress and any changes in condition.
- Maintain records of any conversations, incidents and reports on the Client's feed on the care planning system.
- Experience in Birdie digital care planning system is essential.
- To be able to handle all the phone calls from service users, their families and carers.
- To be able to maintain smooth running and handling of the service on the day.
- To report any incidents, concerns, complaints to the manager and the team leader.
- To be able to handle complains in professional manner and resolve the complains swiftly.
- To be able to handle phone calls in the evenings during the week from 17:00pm to 09:00am. After 09:00am, the office staff will take over.
- To be able to handle phone calls at the weekend and to ensure smooth running of the service.
- To be able to find cover in short notice during crisis or emergency.
- To be able to reshuffle the roster system during crisis or emergency to avoid impairing the service.
Requirements
- Previous experience in senior care settings such as assisted living or home care/community care is essential.
- Familiarity with Alzheimer's, dementia, hospice, and home care practices is highly desirable.
- A background in nursing or caregiving is preferred, along with first aid certification.
- Strong interpersonal skills to effectively communicate with patients, families, and healthcare teams.
- Compassionate nature with a strong commitment to providing high-quality patient care.
Job Type: Part-time
Pay: £112.50 per day
