Are you a Registered Nurse, Therapist, or Practitioner with a specialist interest in supporting patients with Parkinson's Disease ? If so, this could be the perfect opportunity for you

We are looking for a dedicated professional—whether full-time or part-time—who is passionate about making a real impact in our Parkinson's Community Services . If you're currently a Band 6 and looking to advance your career, we can provide additional training to help you gain your V300 prescriber qualification and progress to a Band 7 role .

This is a community-based position, serving the surrounding local areas, with Farnham Hospital as your base . The service operates Monday to Friday, offering flexible hours.

Main Responsibilities
Some of your responsibilities will include:

  • To provide advanced clinical nursing support for patients with complex long-term conditions and/ or elderly/frail.
  • Working as an autonomous practitioner, the post holder will effectively manage a caseload of patients with long term conditions, stratified as high risk.
  • Working in collaboration with appropriate health and social care professionals, the post holder will ensure continuity of care; aiming to reduce preventable hospital admissions and to improve quality of care for patients with long-term conditions in their usual place of residence (own home; residential or nursing home).
  • To engage with innovation, including tele technologies, to continually strive to effectively support the management of patients.
  • To promote self-management strategies for patients and their carers, through education and advice, to reduce avoidable reliance on urgent services and promote positive patient outcomes.

The Ideal Candidate
The ideal candidate will be a registered nurse/therapist or practitioner with at least 3 years experience of working at a Band 6 Level or currently working at a Band 7 Level.

Some experience of working with people with Parkinson's Disease (PD) or long-term progressive neurological conditions and a strong interest in gaining further clinical knowledge and expertise of Parkinson's disease and related disorders.

You will have excellent communication and written skills that will enable you to provide information to support, promote and encourage independence and support decision-making relating to care and support needs.

A UK driving licence is also essential with access to a vehicle to provide our services to the community.
