About this opportunity
We are looking to recruit Social Care Workers to join our Intermediate Care Team based at Maudes Meadow in Kendal.
The service supports residential short-term intervention which provides Intermediate Care and Discharge to Assess (D2A) beds.

You will prioritise and manage assessments and person centred care plans to ensure activities are delivered to an agreed timescale, making recommendations regarding appropriate interventions, evidencing this clearly, and will seek advice and guidance as required.
You will be expected to work in ways that promote independence and enable individuals to maximise their strengths.
