Job Description:
SM Care and Support Ltd is a dedicated provider of semi-independent accommodation for care leavers aged 16 and 17 years old. We pride ourselves on creating a safe, supportive, and nurturing environment that empowers young people as they transition into adulthood. We are now looking for committed support workers to join our team.
Our young people have experienced severe abuse and trauma in their early years. While this is a demanding role, the rewards are like no other. You'll be working in an environment where the small things you do each day can have a huge impact on the lives of the young people in our care. Your dedication and hopes for the young people you support will help them take their first steps towards reaching their full potential.
We focus on giving the best possible support to young people who may have challenging behaviour and mental health needs. Our aim is to create a home environment that goes beyond just meeting their basic needs, where they feel safe, can express themselves freely, and live a fulfilling life.

Job Specification:
As a Support Worker at SM Care & Support Ltd, you will play a vital role in providing high-quality support and care to young people in our semi-independent accommodation. You will assist them in developing essential life skills, promoting independence, and ensuring their safety and well-being. Your role will involve offering emotional and practical support while upholding the values of safety, respect, and empowerment.
Key Responsibilities:
· Provide day-to-day support to young people, helping them develop independence and life skills, including budgeting, cooking, and personal care.
· Build positive relationships with young people, offering emotional support and guidance.
· Ensure that safeguarding policies and procedures are always followed, reporting any concerns to the appropriate person.
· Support young people in accessing education, training, and employment opportunities.
· Encourage and facilitate young people's involvement in decision-making about their lives and their accommodation.
· Assist in developing and implementing personalised support plans tailored to individual needs.
· Promote a safe and secure environment, managing risks effectively.
· Support young people in understanding their rights and responsibilities.
· Maintain accurate and up-to-date records, including incident reports and daily logs/weekly logs.
· Work collaboratively with other professionals, including social workers and external agencies, to ensure the best outcomes for young people.
· Participate in regular supervision, training, and team meetings to enhance skills and knowledge.
· Promote and uphold the values of SM Care & Support Ltd, ensuring a person-centered approach to care.
- Act as a Key Worker, building positive relationships and offering tailored support to young people.
- Promote structure, boundaries, and domestic routines within the home.
- Support young people with project work, group activities, and life skills development.
- Encourage and support young people to access education, training, employment, and extracurricular activities, assisting with applications and enrolments.
- Monitor and assess young people's independent living skills, offering guidance and development support where necessary.
- Escalate concerns, including safeguarding issues, to the House Manager in line with company policies and OFSTED requirements.
- Support young people in managing their physical, emotional, and social well-being, fostering a safe and nurturing environment.
Person Specification:
Essential:
- Experience working with young people in a care or support setting.
- Knowledge of safeguarding policies and procedures.
- Ability to build positive relationships with young people and act as a role model.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- A commitment to promoting equality, diversity, and inclusion.
- Basic IT skills for record-keeping and reporting.
Requirements:
- Level 3 Diploma in Health and Social Care (Children and Young People) or equivalent (required).
- Minimum 1/2 year of experience working with young people in a residential, semi-independent, or leaving care setting.
- Strong understanding of safeguarding, risk assessment, and care planning.
- Ability to promote independence, life skills, and positive engagement.
- Good written and verbal communication skills.
- A proactive and professional attitude with a passion for supporting young people.
- Driving Licence (preferred but not essential).
Shifts & Availability:
- Full-time & part-time positions available.
- Shift work, including evenings, weekends, and sleep-ins on a rota basis.
- Shift options: 8-hour, 10-hour, or 12-hour shifts, 24-hour shifts
