An exciting opportunity to join this friendly and innovative GP Surgery and Training Practice. We require a Care Coordinator who can contribute to our administration team.
Care Coordinator plays an important role within the Primary Care Network, to proactively coordinate activities and provide administrative support to members. The aim of the role is to support the delivery of service improvements and enhanced services within the Practice, leading to improved care outcomes for patients. The Care Coordinators must be self-motivated and able to work independently demonstrating strong organisational and communication skills. To do this, they will work closely with General Practices, Care Homes and existing services across your Primary Care Network .
To help coordinate care needs including the organising and documenting of multidisciplinary meetings within care homes and to support the implementation of personalised care plans.
To utilise Population Reporting within the clinical systems to help identify target patient populations, particularly around cancer screening programmes and childhood immunisations to increase uptake.
To enhance signposting within your practice to ensure colleagues are made aware of any service changes, offering guidance and help as required.
To become a mental health first aider with practices helping support patients and colleagues
Job Responsibilities
Be responsible for arranging, attending, and minuting Multi Disciplinary Team meetings within practice or Primary Care Network including Care Home/ GSF/Safeguarding.
Proactively prepare any actions prior to the MDT ensuring all relevant clinicians are present.
Follow up on all actions resulting from MDT discussions, including record patient interventions on EMIS and contribute to any relevant reports.
Be responsible for daily updating of patients to identity care home residents at risk of hospital admission working with multidisciplinary teams using community pathways that might prevent hospital admission.
Provide coordination and navigation for people and their carers across health and care services, alongside working closely with your practice team, social prescribing link workers, health and wellbeing coaches and other primary care roles.
To help identify and target vulnerable/hard to reach patients to signpost them to appropriate care and services.
To support the development of group consultations within your practice, helping to identify patients who may benefit from this type of consultation.
To work collaboratively with other teams and services, sharing that information with your practice to maintain an effective and efficient service.
To plan/organise work using own initiative, whilst being able to work as a valuable member of a team.
To have IT skills, to include Microsoft Office, Outlook and Excel. Knowledge of the EMIS Web system is desirable; full training and support would be offered to the successful applicant
To use a range of verbal and non-verbal communication tools to communicate effectively with patients, carers and families and colleagues.
To carry out any other reasonable duties as requested by Management to ensure quality of service.
To participate in any relevant training required for the role
