Are you passionate about transforming care and optimising patient outcomes for individuals? Join our dedicated team across Dartford, Gravesend and Swanley, North Kent as a AFC Band 7 Community Clinical Specialist Nurse

In this vital role, as a Clinical Specialist Respiratory Nurse, will work as an independent practitioner delivering high quality Respiratory Nursing care to a specific caseload of patients, improving their quality of life, promoting excellence of health and independence within the community, preventing inappropriate hospital admissions, and facilitating early discharge, as well as provide education and health promotion.

Be the Clinical Lead for the service and support the achievement of clinical and non-clinical performance in conjunction with the Clinical Service Manager.

An exciting opportunity to work collaboratively with primary care to support active case finding for COPD patients achieved with partnership working with the acute trust aligning to the neighbourhood health model.

Your base will be Livingstone Hospital, Dartford, with the requirement to travel across Dartford, Gravesend and Swanley. As this role is community based, a UK driving license and access to a car for work purposes is a requirement.

Main Responsibilities

As a Clinical Respiratory Nurse at HCRG Care Group, you are responsible to provide specialist care for patients with chronic respiratory conditions like COPD and asthma.

Key responsibilities include

  • To lead on Clinical Governance for the service and support the achievement of clinical and non-clinical performance measures, in conjunction with the Clinical Service Manager.

Assessing and managing patients, delivering clinical interventions, educating on self-management, and monitoring progress to prevent hospital admissions.

  • Work closely with multidisciplinary teams, support end-of-life care, and contribute to service improvements.
  • Compassion, clinical expertise, and excellent communication are essential for this role.
  • Provide care, advice, and guidance in the treatment of others, when dealing with complex highly specialist respiratory conditions.
  • Acts as a specialist advisor across primary, secondary, and tertiary care settings, supporting communication and care continuity.
  • Contributes to service vision, growth, and transformation, aligning practice with national guidelines and local healthcare strategies

The Ideal Candidate

A Respiratory Specialist Nurse will:

  • Be a Registered Nurse with a valid NMC Registration.
  • Minimum of 2 years' Respiratory nursing experience working at Band 6.
  • Masters or equivalent post graduate qualification in Respiratory care (certificate)
  • Evidence of CPD in Respiratory O2 Care and Management
  • Recognised teaching certificate e.g. ENB 998
  • Non-Medical Prescribing V300 or willing to work towards
  • Knowledge of NICE, and excellent understanding of COPD, ILD, ACOS, and Bronchiectasis
  • Ability to carry out oxygen Assessments
