About The Role
As our Out of Hours Care Coordinator, you'll be the heartbeat of community care during evenings and weekends, guaranteeing seamless, high-quality support for our users. You will coordinate care schedules, build strong communication channels with families and carers, and make critical decisions that impact people's lives — all while working in a passionate, supportive team committed to excellence.

What You'll Do
Lead with Compassion & Precision:

  • Manage and optimize care schedules and rotas, tailoring services to each individual's needs and preferences.
  • Act as the go-to contact during evenings and weekends, handling emergencies with calm expertise.
  • Engage proactively with users, families, and carers, ensuring everyone's needs are met and expectations are exceeded.
  • Work with internal and external health professionals to facilitate smooth home visits and integrated care.
  • Organize training, quality assurance visits, and team development opportunities to elevate our service standards.
  • Collaborate closely with the Care Team to ensure everyone is supported and informed.
  • Participate in interviews and onboarding, helping us find and grow our outstanding care team.

Keep Everything Running Smoothly

  • Monitor attendance and scheduling, ensuring every visit is completed on time.
  • Communicate effectively about absences and schedule changes to ensure no one is overlooked.
  • Maintain detailed records and documentation that support quality, safety, and compliance.
  • Share user feedback, report incidents, and contribute ideas for continuous improvement.
  • Identify opportunities for business growth and enhanced service delivery.

Champion Safety, Dignity & Respect

  • Uphold all policies related to safety, confidentiality, and safeguarding.
  • Escalate concerns immediately to protect our service users and staff.
  • Ensure all practices Respect human rights, promote independence, and preserve dignity in every aspect of care.

What You Bring

  • Experience in care coordination or a similar healthcare role.
  • Exceptional communication and empathy skills.
  • Calmness under pressure and quick decision-making abilities.
  • Strong organizational and problem-solving skills.
  • Knowledge of healthcare regulations and best practices.
