We currently have opportunities for Community Band 6 nurses to join our team, working in the London Borough of Lewisham. We cover all areas of the Lewisham Borough and the role could be based in any one of our 4 neighbourhoods.

You might already be a band 6 looking for your next role, or an experienced band 5 nurse with hospital or community experience looking to develop your skills and knowledge. Either way we can support you with a robust training package which will give you the skills you need to succeed.

The successful candidate will be experienced in caring for patients with a variety of long term conditions and, with a desire to develop their leadership skills. Applicants will have the opportunity to apply their knowledge and evidence-based practice to community care.

The aim of our service is to enable patients to stay at home longer, and leave hospitals faster, maximising their potential for independence and wellbeing.

As no two days are the same in the community, you will be working with a large degree of autonomy and accountability, with the care you are giving makes a real difference to your patients and those important to them.

  • To be accountable and responsible for the management of a district nursing team and the delegation of workload as appropriate
  • To supervise a skill mix team of nurses, assistant practitioners & health care support workers to ensure
  • To manage the capacity and demand of the workload efficiently, economically and escalate any concerns appropriately
  • To act as a role model and motivate team members
  • To facilitate future learning for staff and students
  • To prioritise and assist in the investigate and manage of complaints according to Trust policy
  • To maintain accurate and complete records of activities and communications relating to patients in a timely manner in line with Trust and NMC standards for record keeping
  • To actively participate in the recruitment and selection process
  • To ensure all new staff, temporary staff and student nurses are inducted and orientated appropriately
  • To contribute to any audits/research/benchmarking as required
  • To record all 1-1 meetings and communication regarding staff management and performance ensuring HR support where necessary
  • To communicate clear expectations to staff & colleagues, manage performance and champion change to create an environment where the courage to speak out is welcomed and encouraged.
  • To manage staff absences and conduct return to work interviews escalating all performance concerns in line with Trust HR policies and procedures
  • To ensure that all staff are up-to-date with mandatory and statutory training, appraisals and personal development plans
  • Act to safeguard vulnerable children and adults in the community
  • Report and escalate all incidents and near misses in accordance with Trust Policy
