Job Title: Adult Social Worker – Mental Health Team
Location: Bolton
Rate: £30 per hour
An exciting job role has recently become available at Bolton Metropolitan Borough Council. They are looking for a new member for their Mental Health Team.
What will your responsibilities be?
In this role, you will be responsible for providing a purposeful, professional social work service to appropriate clients. This includes conducting comprehensive assessments of complex health and social care needs, performing risk assessments and management, and delivering care and interventions in line with statutory requirements and Departmental policies. You will undertake the role of a Care Coordinator, and follow the Care Programme Approach (CPA), working collaboratively and communicating effectively with other professionals, agencies, organizations, and volunteers. This ensures that community resources and support are available to service users and their carers, helping them maintain or improve their quality of life. Additionally, you will ensure that all carers are offered a full carers assessment and, where appropriate, a care plan that identifies their needs and how they may be met.

Qualifications and Experiences:
To be eligible for this role, you need to have Social Work England registration and be eligible to working the UK. You must also hold a full UK driving licence and have post-qualified experience. Additionally, you should possess a degree level or equivalent qualification in Social Work.
