A member of the bladder and bowel service who is skilled in providing and monitoring nursing care.

The post holder always acts professional and use their knowledge and experience to make evidence-based decisions and solve problems. They recognise and work within the limits of their competence and are responsible for their actions.

They contribute to the provision of care for people, including those with complex needs. They understand the roles of a range of professionals and carers from other organisations and setting who maybe participating in the care of a person and their family, and their responsibilities in relation to communication and collaboration.

The post holder will improve the quality of care by contributing to the continuous monitoring or peoples experience of care. They identify risks to safety or experience and take appropriate action, putting the best interests, needs and preferences of people first. They will contribute to on-going assessment and can recognise when it is necessary to refer to others for reassessment.

The role requires the ability to be flexible and adaptable whilst working in peoples homes and other community settings.

The post holder will have some responsibility for supervising less experienced or qualified staff and students in community placements and can act as a supervisors and/or assessors for Trainee Nurse Associates. They will be expected to actively contribute to quality assurance processes and service development.

To assess physical and psycho–social needs, plan, manage, implement and evaluate individual programmes of patient care in the most efficient and clinically effective way, which meets the needs of the patient and his/her carer(s).

Uses a range of assessment tools pertinent to the person's need to inform the assessment and assess risk for both patients and staff. Performs continence assessments.

Understands when to seek appropriate advice to manage a risk and avoid comprising quality of care and health outcomes acting in line with local and national organisational frameworks, legislation and regulations to report risk, and implement actions as instructed.

Demonstrates an ability to prioritise and manage their own workload and recognise where elements of care can safely be delegated to other colleagues carers and family members

Works in close liaison with nursing colleagues, of members of the multidisciplinary team and social care and voluntary sector.

Contributes to the provision of care for people including those with routine non problematic urinary catheters and including those with complex needs that have been taught intermittent self catheterization by a registered nurse. To perform second attempt at trial without catheter patients known to be uncomplicated

Exercise professional accountability in ensuring the safe administration of medicines in line with trust policies and those receiving care

Ensuring information is recorded objectively and reported back to the Bladder & Bowel Service

Recognise and respond to adverse or abnormal reactions to medications, and when and how to escalate any concerns to coordinator/ team leader.

Undertakes storage, and disposable disposal of medical products in line with trust policies

Ability to plan ahead for potential scenarios to ensure anticipatory care needs are understood and met.

Is able to show an awareness of the roles, responsibilities and scope of practise of different members of the nursing and inter multidisciplinary team and their own role within it

Ability to recognise when a person's condition has improved or deteriorated by undertaking health monitoring interpret, promptly respond, share findings, and escalate as needed

Collaborate effectively with other members of the multidisciplinary team or other agencies involved in the persons care. Offers support with training in the care home environment for healthcare support workers.

Participates in case conferences, MDTs and care package reviews providing health related information when requested.

Recognise and apply knowledge of commonly encountered mental, physical , behavioural and cognitive health conditions when delivering care full stop additionally demonstrate and apply an understanding of how and when to

recognise to escalate to the coordinator.

Support supervise and act as a role model to nursing associates students healthcare support workers and those knew to care roles review the quality of their care they provide promoting reflection and providing constructive feedback

Work in partnership with people, to encourage shared decision making, in order to support individuals, their families and carers to manage their own care when appropriate. Whilst minimising the risk for both people and staff making use of all available resources

Provides support and education in the delivery of care and delegates work appropriately to junior team members. Undertakes staff appraisals, when appropriate.

Promotes and applies the aims and principles of health promotion protection an improvement and the prevention of ill health when engaging with people

Protect health through understanding and applying the principles of infection prevention and control including communicable disease surveillance and antimicrobial stewardship and resistance.

Encouraging and support patient independence and self-care of their condition where appropriate. whilst minimising risk for both patients and staff, making use of all available resources.

As part of promoting a safety culture recognise an report via datix, any situations behaviours or errors that could result in poor care outcomes

Takes reasonable precaution to ensure that the patient and the patient's property is safeguarded.

Is involved in audits and distributes and encourages completion of patient experience questionnaires

Assess clinical and environmental risk and share with appropriate services.

Maintains good working relationships with colleagues and other agencies. Understand the demands of professional practise and demonstrates how to recognise signs of vulnerability in themselves or their colleagues and the action required to minimise risk to health

Is required to communicate effectively with patients, carriers, relatives and other professionals

Recognises personal accountability and responsibility to monitor and evaluate care to ensure optimal practice.

Understands and works within NMC professional codes of conduct and practice, maintaining own professional development and competence to practice . Maintaining revalidation and registration.
