An exciting opportunity has arisen for a new role as a Band 6 Senior Locality Hub Clinician . The Hub Clinician will assess the new referral or message to determine when/ if the patient requires a visit . You will act as an autonomous practitioner providing a comprehensive telephone triage and clinical assessment service to patients and/or their carers/relatives resulting in a safe and appropriate clinical decision. As a Senior Hub Clinician you will play an integral role in supporting a system wide approach, prioritising care in the community and prevention.
This post will be a locality role covering different community nursing teams on a rota basis.
The Clinical Hub Coordinator, under the supervision of the Clinical Team Leader, manages the coordination of patient care within the locality, by triaging referrals and messages to ensure patients are on the correct clinical pathway, and in the undertaking of clinical hub functions including in-reach into the aligned acute hospital.
The post-holder will be accountable for the delivery of Safe, High-Quality care to patients within the locality across various settings.
The Clinical Hub Coordinator (Registered Nurse) role is based within the Locality Hub working as part of an integrated health and social care team to support the co-ordination of patient care across the Locality. The Locality Hub co-ordinates care across acute, community, and primary care settings, reducing length of stay and avoiding unnecessary hospital admissions, focusing on intervention and prevention to meet the local needs of the population within the locality.

Work in accordance with the NMC Code of Professional Conduct, scope of Professional Practice, Trust Policies and practice in an evidence-based manner.
To provide a holistic approach to patients/clients that includes clinical assessment and skills in triaging referrals and messages, to evaluate the information given, using appropriate questioning, in order to establish the correct clinical pathway based on the clinical service and its priority.
Demonstrate critical thinking and assessment skills when making clinical decisions,
during the assessment of a variety of patients, some of which may be complex and require an MDT approach.
To act as an autonomous practitioner providing a comprehensive triage and clinical assessment service ensuring safe and appropriate clinical decisions are made. Referrals and messages may come from clinicians, patients or their representatives and given the complexity of some cases, there may be barriers to understanding which the post-holder will be required to deal with in line with Trust values. There will also be a requirement to deal with very emotive and sensitive situations in regard to patient care.
The post-holder will undertake the locality clinical in-reach function, in collaboration with colleagues at the aligned acute hospital. This will involve the identification of patients with community amenable presentations who could be discharged from hospital with the appropriate community support. By liaising with acute, community, primary care and social care teams to initiate ongoing referrals and integrated care pathways, the post-holder will support flow between services, ensuring a seamless provision of care.
To support the monitoring and triage of alerts for patients residing in locality care homes through the electronic remote monitoring system, identifying and initiating the correct clinical pathway and provision of follow up care.
To utilise clinical systems and gather relevant information to support decision making, this will require prolonged concentration.
Obtain appropriate input and direction from internal and partnership clinicians and professionals as needed to determine best clinical course of action and recommendations for specific patient healthcare needs.
Participate in other locality hub functions to ensure efficient, effective and timely responses to the needs of patients, through reducing hospital Length of Stay (LoS), preventing hospital admissions and improving overall patient care and safety.
To provide advice and support to other clinicians in the delivery of patient care.
Work in a manner which ensures that patients, carers and colleagues are treated fairly, according to their needs, in line with Trust Equality, Diversity and Inclusion policies.
To contribute to the protection of individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately and informing the senior clinician on duty. The post holder will frequently be exposed to distressing situations.
To maintain accurate clinical records and documentation, ensuring that the NMC Code of Conduct is adhered to.
Deputise in the absence of the Team Leader to co-ordinate the day-to-day management of locality hub functions.
Participate in proactive service user engagement encouraging feedback and evidence actions taken in response to feedback received.
To undertake other duties commensurate with this grade of post in agreement with the relevant line manager
