DUTIES AND RESPONSIBILITIES Clinical 1) Triage and action Haematology CNS patient calls. Triage individuals contacting the service face to face or by telephone/ email and work autonomously to prioritise needs and initiate appropriate response according to protocols and individual pathways.2) Coach patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours. Coach patients and carers to understand what signs, symptoms, or situations to be aware of that would indicate concern3) Collection of Audit data for patient satisfaction surveys and disease specific quality of life surveys.4) Assist in providing evidence based individualized patient care according to patients changing healthcare needs, through holistic needs assessment. To be able to recognise the importance of the psychological and emotional needs of patients and their families.

Undertaking eHNA independently.5) Liaise with external centres, health services to coordinate local bloods tests and results in preparation for patient clinic appointments.6) Work within multi professional meetings. Represent the CNS team at weekly ward MDT to discuss patients social, economic, and holistic needs. 7) Supporting departmental flow and coordination of patient care through the relaying of information, patient appointments, scheduling requests. 8) Preparation of Bone marrow biopsy room, gathering, organising equipment for CNS Bone marrow list.

9) Follow departmental protocols and check Phlebotomy results, Identifying low results, discussing with medical team, coordinating transfusions slots, and organising cross match samples for processing. 10) Sample collection for external research as part of the transplant patient follow up/monitoring.11) Support the CNS team to assess patient`s information needs and provide relevant information to meet those needs, ensuring the focus is on the promotion of health as well as illness. Sign posting and referring patients to hospital and external services when necessary12) Liaison and completion of referrals to community services, palliative care teams, DN teams and Gps.13) In times of service need/staff leave to assist the Transplant support assistant role in clinical duties. Perform specific recognised nursing clinical skills on instruction/direction, with direct/indirect supervision from the qualified nurse, following completion of training and assessment of the recognised skill.

These include: o Take and record patients observations including height and weight recording/reporting any abnormal results.o Take peripheral bloods and blood samples and record/report abnormal results.o Take bloods from CVADs.o Care of central lines including swabs dressings as required.o Assisting with sepsis screen as required.o Perform peripheral cannulationo Flush peripheral cannula and central lineso Set up & connect patients to monitoring systemso Perform ECGso Remove peripheral cannulas & PICC lines under supervision Non Clinical 14) Demonstrates the agreed set of values and accountable for own attitude and behaviour 15) Recognise and be able to respond appropriately to challenging behaviour.16) To be aware of limitations and utilise wider team for any issues that arise. 17) Assist in supporting the CNS team in the preparation and completion of BM clinics. Prepare trolleys for specific clinical procedures in line with trust policy utilising aseptic non touch technique practices. Education Attend annual updates and ensure compliance with all mandatory training Take responsibility for learning opportunities and personal development through appraisal and actively engage with learning development opportunities.

Support the delivery of patient and carer training and education. Encourage and support active and healthy lifestyle choices Professional Development Support the haematology CNS team with service development, ensuring the pursuit of excellence in care Contribute to effective team working, by being fully involved, committed and participative to achieving full potential. Demonstrate respect for the individual patient, ensuring religious, cultural and social needs are always considered and acknowledge their individual rights. In all situations ensure consideration is made to the patients cultural background.

Undertake all training required to perform duties effectively to the requirements of the post. Attend all Mandatory training courses. Maintain own professional portfolio in accordance with personal development plans. Ensure the provision of care is in accordance with The Christie NHS Trust policy and procedures guidelines Promote patient centred services for patients affected by cancer and work in collaboration with allied health professionals in the trust and in the community
