Undertake structured medication reviews (SMRs) in line with the contractual requirements of the PCN DES for targeted cohorts of patients that would most benefit, including those most at risk of experiencing health inequalities. Employ medicines optimisation strategies for reducing polypharmacy, minimising risk of prescribing harm, reducing over-prescribing and managing the risk of dependency on prescription drugs. Be a prescriber, or in the process of completing the training to become a prescriber, working alongside the Practice clinical team. Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local or national guidance.

Provide clinical leadership on medicines optimistion, including high-risk drugs, repeat prescribing and deprescribing Support the management of patients with chronic diseases such as diabetes, asthma, hypertension and heart failure, in line with national and local guidelines. Including seeing patients in Chronic Disease clinics, starting and titrating medicines. Proactively manage medicines-related risk, including contributing to medicines safety and incident investigations. Full job description is attached as an additional document.
