  • Location: Exeter / Hybrid Working (Mix of remote, prison, and community).
  • Contract: Full-Time, Part-Time, or Flexible Working (Monday–Friday).
  • Closing Date: 5pm, 14th November 2025.
  • Role Purpose: To ensure a seamless transition for individuals released from custody.

The Role: Intensive Transition Support

You will provide intensive, person-centred support for both men and women, focusing on recovery and reintegration.

Key Responsibilities

  • Manage a personal caseload.
  • Provide 12 weeks pre-release engagement and 12 weeks post-release support.
  • Complete strength-based assessments and deliver harm reduction interventions.
  • Coordinate release day logistics (transport and support).
  • Build and maintain effective partnerships with Housing, Probation, and Drug & Alcohol providers.

Essential Experience & Skills

This role requires compassion, resilience, and specific knowledge of:

  • Working within the Criminal Justice System (Prisons, Courts, Probation).
  • Supporting individuals affected by substance use and recovery.
  • Experience in multi-agency/partnership environments.
  • Strong caseload management, assessment, and communication skills.
