Overview

We are seeking a dedicated and compassionate Care Manager to oversee the delivery of high-quality care services in the community. Duties

  • Develop and implement personalised care plans for customers, ensuring their needs are met with dignity and respect.
  • Oversee daily operations within the branch, ensuring compliance with regulatory standards and best practices.
  • Coordinate with healthcare professionals, families, and staff to provide comprehensive support to customers and their families.
  • Conduct regular assessments of customers' health and well-being, adjusting care plans as necessary.
  • Provide training and support to staff members, fostering a culture of continuous improvement in care delivery.
  • Maintain accurate records of care provided, ensuring confidentiality and adherence to data protection regulations.
  • Manage budgets and resources effectively to ensure the sustainability of care services.

Skills

  • Proficient in English with excellent verbal and written communication skills.
  • Strong IT skills for maintaining records and managing care plans effectively.
  • Experience in home care or within a care home setting is highly desirable.
  • Ability to drive is essential for transporting customers as needed and fulfilling home visits.
  • Exceptional organisational skills with attention to detail in all aspects of care management.
