CKD Clinical Nurse Specialist

An opportunity has arisen for a dynamic and self-motivated nurse to join the chronic kidney disease (CKD) team for a 10 months fixed term contract.
This is an exciting role working within an established team alongside our wider multi-disciplinary care providers allowing us to provide a high level of support to both patients and their families across Devon and Cornwall.
You will be required to review patients in a variety of clinical settings; these include but are not limited to both the outpatient and inpatient setting therefore ability to travel between sites is essential and the ability to adapt your clinical caseload accordingly.
We are looking to recruit a registered nurse who has demonstrable experience in the renal setting as there is a strong commitment to education and health promotion by adhering to regional and national strategies and guidelines with emphasis on advanced practice within this role.

On call work from home is part of the registered nurses role where the CKD nurse will be expected to provide a prospective 3 in a six week period, once established into the role.
Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS at Risk staff throughout Devon who is able to display recent relevant experience as dictated by the Person Specification.
Management of the CKD team in the absence of the Band 7
Ensure cross cover is provided for the patient caseload when colleagues are on leave
Undertake the appraisals and sickness management of junior colleagues
Undertake nurse led CKD clinics on site and at peripheral hospitals
In the absence of the team lead to be responsible for ensuring resources are used effectively within
the clinical environment
Assist the transplant team with the on call transplant co-ordinator role
Undertake home visits when required

- To ensure all CKD patients starting renal replacement therapy have had all the appropriate education
to allow them to have an informed choice of their options for renal replacement therapy
Mange the CKD team in the absence of the manager
Undertake the co-ordination and the management of CKD patients with renal anaemia
Implement individual education programmes for patients and carers entering the pathway
Assist with continuing development of health promotion for CKD patients
Undertake the continuing development of the End of Life care (EOL) for CKD patients
Constantly updating one own knowledge , recognising the importance of a critical approach to research
and developments in nursing
Be actively involved in research & clinical audit within the clinical area of CKD/renal
Assist in identifying all potential recipients for transplantation work these patients up for referral
Participate in the transplant on call rota
Work collaboratively within the multidisciplinary team and participate and participate in the induction of
new staff to renal
