APT Care Ltd is expanding, and we're looking for a dedicated Field Care Supervisor based in Cambridge to support our growing team. This is a community-focused role, ideal for someone passionate about supporting others. You'll work closely with our care staff, providing training, coaching, and mentorship to help them achieve their full potential. You'll also act as a key point of contact for service users, families, and other health and social care professionals to ensure the highest standards of care are maintained. Key Responsibilities:

  • Conducting home visit assessments following hospital discharge
  • Attending professional meetings and handling email/telephone communications
  • Creating short-term care plans to support hospital discharges
  • Carrying out annual reviews and updating care plans
  • Assessing the competency of community carers
  • Signing off new carers through our 'buddy supervisor' process
  • Supporting management with investigations and follow-up actions
  • Delivering medication and manual handling training (for new staff and annual refreshers)
  • Making referrals to other professionals (e.g. SALT, GP, nurses, safeguarding, CQC)

Job Types: Full-time, Permanent

Pay: £13.66 per hour
