Join our dynamic team as a Social Prescriber Team Lead, where you will play a pivotal role in guiding a dedicated group of social prescribers, Care Coordinators and Health & Well Being Coach to deliver impactful, person-centred support. As a vital connector within the community, you will foster collaborative relationships with healthcare providers, social services, and community organizations to promote well-being and resilience. Responsibilities

  • Lead and supervise a team of social prescribers, providing mentorship, coaching, and performance feedback to foster professional growth.
  • Oversee client engagement processes, ensuring timely assessments and personalized care plans that incorporate crisis management and behavioral health techniques.
  • Coordinate with healthcare providers, child welfare agencies, and community partners to facilitate comprehensive support services aligned with clients' goals.
  • Implement crisis intervention strategies effectively during urgent situations, including addiction counseling and mental health crises.
  • Conduct regular team meetings to review caseloads, share best practices in motivational interviewing, and ensure adherence to organizational policies.
  • Develop training programs focused on social work principles, workforce development, and specialized areas such as Section 8 housing or Medicare navigation.
  • Monitor documentation accuracy and compliance with confidentiality standards while maintaining detailed records of client interactions and progress.

Qualifications

  • MUST have proven supervising experience leading social or behavioral health teams in community or healthcare settings.
  • Strong background in crisis intervention, crisis management, addiction counseling, and behavioral health practices.
  • Knowledge of social work principles, child welfare systems, Section 8 housing programs, Medicare policies, and workforce development initiatives.
  • Excellent skills in motivational interviewing and mentoring to motivate clients toward positive change.
  • Ability to handle sensitive situations involving childcare issues or complex mental health challenges with professionalism and compassion.
  • Effective communication skills for collaborating with diverse stakeholders including healthcare professionals, social service agencies, and community organizations.

Pay: £15.00-£20.00 per hour
