Job Overview
The Community Independence Service in Kensington and Chelsea is looking for a motivated and enthusiastic Band 6 Physiotherapist to join our friendly Community Rehabilitation Team. You will be passionate about the value of community and rehabilitation, have had clinical experience and be ready to step into a community role to further develop your clinical and non-clinical skills.

The community setting is THE PLACE TO BE with care being prioritised at home and patients being cared for in the community wherever possible - this is clearly outlined in the NHS Long Term Plan and can already be seen on the ground.

We are part of a wider Community Independence Service, which is also offered in Hammersmith & Fulham and Westminster. You will be connected with peers across the tri-borough to support you and to work on service improvement partnerships. You will also be working closely with our other CIS teams, and acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.

Main duties of the job

The Community Independence Service (CIS) is a provider partnership which focuses on hospital admission avoidance; where clinically appropriate care can be provided in the community, facilitation of early supported discharge from hospital, maximising independence and reducing dependency on longer term services. The band 6 post holder will see clients newly discharged from hospital, use evidence based/patient centred principles to assess, plan and deliver individual treatment plans. The post holder will undertake all aspects of clinical duties as an autonomous practitioner, with supervision from senior therapists within CIS.

The post holder will be expected to promote, maintain and develop a high quality, efficient and effective physiotherapy service to clients in accordance with the philosophy and operational policies of the team. The post holder will be encouraged to develop skills in providing undergraduate placements to students.

You will deliver skilled physiotherapy care to clients in their own home, based on a thorough holistic assessment of their clinical and personal needs and the agreement of an individualised rehabilitation care plan.

Person specification

Education And Qualifications
Essential criteria

  • Degree/ Diploma Physiotherapy
  • Registration with HCPC as a Physiotherapist

Desirable criteria

  • Member of CSP
  • Post registration courses s e.g. Neurology, Older people/elderly rehab , Goal setting , Student Placement Educator
  • Membership and/or involvement in a CSP professional network
  • Post graduate MSc degree

Previous Experience

Essential criteria

  • Previous experience of working in the UK or a similar health economy as a physiotherapist.
  • Previous general experience in older people/elderly rehab, orthopaedics, general surgery, falls, dementia, respiratory
  • Experience of teaching juniors/rehab assistants
  • Experience of completion of audit and service developments

Desirable criteria

  • Experience of working as an autonomous practitioner in a community setting
  • Previous general experience to include the following: neurology, community working, pain management, group work
  • Experience of teaching students

Skills and knowledge

Essential criteria

  • Excellent communication skills, fluency in written and verbal English
  • Demonstrates flexibility, problem solving, initiative and leadership skills
  • Able to demonstrate clinical and professional development
  • Understanding the principles of Multi-Disciplinary Teamwork
  • Understanding of the problems of disability and its effects

  • Able to demonstrate a good understanding of role of the physiotherapist in rehabilitation as well as the role of other professionals in MDT

  • Able to demonstrate an understanding of the application of Health and Safety
