Job Summary

Chadwell Healthcare Ltd is seeking a compassionate and dedicated Outreach Family Support Worker to support children, young people and their families across Liverpool. Working within the Early Help framework, you will provide home-based and community outreach support (Level 2–3), helping families to build stability, reduce risk and improve outcomes.

Responsibilities

  • Hold and manage a caseload, including cases stepped down from Children's Social Care
  • Complete Early Help Assessments and lead Team Around the Family (TAF) planning
  • Visit families in their homes to deliver targeted support and practical parenting strategies
  • Support children and young people aged 0–25 (SEND up to 25)
  • Maintain accurate electronic records, plans and chronologies
  • Work in partnership with schools, health teams, Children's Centres and other agencies
  • Promote safeguarding, follow Signs of Safety (SOS) principles and ensure all work meets Early Help Practice Standards
  • Contribute to supervised contact and multi-agency meetings where required

Qualifications

Essential:

  • NVQ Level 3 (or equivalent) in Childcare, Education, Health or Social Care
  • Experience providing direct support to children and families
  • Knowledge of safeguarding, child development and Early Help processes
  • Strong communication, record-keeping and multi-agency partnership skills
  • Ability to work both independently and as part of a team

Desirable:

  • Experience delivering parenting programmes
  • Understanding of the SEND Code of Practice

Additional Information

  • Enhanced DBS is required
  • Office base: Clubmoor Children's Centre
  • Must have the Right to Work in the UK – this role does not offer sponsorship
  • Flexibility is required to meet service needs
