JOB PURPOSE To work as manager of a team of nurses within the Parklands Medical Practice identifying and managing patients with complex needs using highly specialist nursing knowledge. 2.

PRIMARY DUTIES AND AREAS OF RESPONSIBILITY KEY ROLE - MANAGE HEALTH INTERVENTIONS The Practice Nurse Manager is expected to work together with their General Practitioners, District Nurses, Social Services and other primary and secondary care colleagues to deliver programmes of care in specialist clinics to a caseload of patients with complex needs. Support and guidance on areas of professional activity, and service development are provided by the GPs with special interests. The post is managed rather than supervised. The PNM will act as providers of care, taking responsibility for ensuring all health and social care needs are met so that the patients condition stays as stable as possible and wellbeing is increased.

The PNM is accountable for their own professional actions and will be required to: Use high-level assessment skills to identify individuals needs which affect their physical, mental and social well-being. Review medication and arrange for medications to be prescribed as required. Provide clinical care and health promoting interventions. Teach and educate patients and their carers how to identify subtle changes in a patients condition that may indicate acute exacerbation of an underlying condition.

Be seen by colleagues as having the key role as care co-ordinator for patients with complex chronic disease conditions. Develop therapeutic relationships with patients, promoting the principles of self-care and contingency management. Undertake and refer patients for a range of physical and functional tests and assessments, in order to inform decision-making and care pathway development. Monitor indicators of long term conditions, anticipating possible decline and proactively managing this to ensure optimum wellbeing and independence.

Assess risk which affects the health and safety of individuals. Update patients medical records, and inform other professionals about changes in condition. Plan, implement, monitor and review specialised programmes of health interventions for individuals. Promote peoples equality, diversity and rights.

Support people to manage disability, loss and change. Contribute to the protection of individuals from abuse and raise any safeguarding concerns. Work positively with patients and carers, taking account of their expressed wishes. Prepare patients and their families for changes in condition.

Evaluate outcomes in collaboration with GP. Work within relevant legislation for example Health and Safety, NMC code of professional conduct and NICE guidelines. Work within all relevant practice policies and procedural guidelines. KEY ROLE - MANAGE clinical nursing Provides leadership and support for other members of the primary health care team, and leads the co-ordination of team activities as relevant to clinical nursing.

The PNM will work independently using own initiatives and is accountable for their own professional actions for delivering highly quality care in all areas of nursing relevant to primary care Develop, monitor, evaluate and review processes for delivering a care service to a defined caseload within a practice population, working collaboratively with GP and practice colleagues and other agencies to ensure a multidisciplinary approach to care. Work in partnership with GPs sharing information and planning together. Develop, sustain and evaluate collaborative approaches with colleagues to achieve objectives. Evaluate, prioritise and review demands for services ie audits outcomes of care against standards and initiates change as necessary.

Oversee audits done by the nursing team and collate findings to effect change as required. Contribute to the practice compliance with the Health and Social Care Act (2008), as assessed by CQC inspection. KEY ROLE - PROMOTE HEALTH / PUBLIC HEALTH Enable, encourage and support individuals and families to address issues which affect their health and social well-being. Provide information so patients and families can make choices about current and future care needs.

Play an active role in raising public awareness and promoting the needs of patients with chronic disease and other defined health issues. Identify and deliver training / talks / lectures to groups as required. Act as an advocate for patients as appropriate. KEY ROLE - MANAGE / LEAD SELF / OTHERS The PNM has a role in promoting good leadership for all grades of staff as well as all students including medical students.

Develop ones own knowledge and practice to meet objectives / changes in service. Promote effective communications and relationships within the primary healthcare team and wider organization. Work collaboratively with General Practitioners, Nurse Practitioners, Practice Nurses, Social Services departments and others to plan and implement care programmes for the management of long term conditions and other health needs. Contribute to the development of staff and students to enable them to meet objectives.

Partake in clinical supervision for own development. Undertake training for other nurses on specific aspects of chronic disease, long term conditions and other health issues relevant to primary care. Take responsibility for the development and training of members of the nursing team ensuring high standards of care are maintained at all times. KEY ROLE - INFLUENCE AND DEVELOP POLICY Contribute to the setting of policies which will affect health and social well-being services in the Primary Care setting.

Write protocols / care pathways and procedures for SPNs service. Implement agreed practice policies and BDCT policies where appropriate within own service area. KEY ROLE - MANAGE RESOURCES Plan, evaluate and recommend the use of resources. Use and maintain resources appropriately.

.No financial / budgetary responsibility required for this post. KEY ROLE - MANAGE AND USE INFORMATION Use data to actively seek out patients who will benefit from chronic disease management. Obtain, analyse, share and use current information for decision making. Maintain high standards of record keeping.

Make recommendations to raise standards of practice and service delivery. Provide accurate and timely information as requested by the Practice. KEY ROLE - RESEARCH AND DEVELOPMENT Critically evaluate research and integrate theory into best practice so that all care and clinical decisions are based upon sound evidence. Regularly read and be able to critique research / evidence-based papers.

Contribute to CCG / service audit and research activity as required. WORKING CONDITIONS Working hours are between Monday and Friday, with occasional Saturday mornings on a rota basis. Frequent exposure to body fluids, blood and wounds. Occasional exposure to aggressive behaviour.

Working to all practice and CCG policies and guidelines. Frequent requirement to travel between sites Occasional requirement to perform home visits. PAYBAND: Agenda for Change Band 6 or 7 METHOD OF PAYMENT: Monthly by bank credit transfer HOURS OF WORK: to be confirmed SUPPERANNUATION: This post is optionally superannuable.
