South Derbyshire Short Term Assessment and Reablement Team (START)

There is an exciting opportunity for a Service Manager to help lead the South Derbyshire Short Term Assessment and Reablement Team (START) based at the area office in Swadlincote, working closely alongside a job share Service Manager.

We are seeking a highly skilled and driven individual with excellent leadership and management skills who can inspire, motivate, and maintain high levels of service performance and professional standards. This will be delivered through strength based, personalised, goal orientated outcome focused approaches and in an integrated and collaborative way with our system partners and other Adult Care teams. You will be ambitious for success, setting targets, monitoring performance, and ensuring high-quality standards are maintained whilst providing supervision, caseload management oversight, and Performance Development Reviews. The role also includes the management of departmental budgets and providing specialist advice and guidance in relation to legislation and quality improvement.

You will be managing a fast-paced, short term specialist team focused on the wellbeing, empowerment and reablement of people in our community, enabling people to remain in their own homes for a long as possible. We aim to prevent the need for hospital admissions and entry into longer term care settings through prevention and timely interventions alongside our system partners. We also enable people that are stepping down following a stay in hospital. We support people to achieve their full potential, to live as safely, independently and as well as possible in their own homes. We take a multi-disciplinary approach, and the team is a mixture of Senior Practitioner, Social Workers, Social Care Practitioners, Occupational Therapists, Enablement Support Team Manager (registered manager), Enablement Support Leads and Enablement Support Workers. We work closely alongside our health colleagues in an integrated way.

Experience of managing a variety of Care Act interventions with a focus on personalised strength-based approaches and outcomes is essential. You will produce performance reports and other reports in the course of your work, chair meetings, and deputise for the Group Manager when required. You will have an overview of the whole team and will be overseeing Safeguarding enquiries, MARM, Mental Capacity Assessments and Best Interest Decisions.
