We are seeking a compassionate and proactive Care Co-ordinator to join our dedicated healthcare team. Duties and Responsibilities

  • Coordinate client care by liaising with healthcare providers, patients, and families to ensure seamless service delivery.
  • Maintain accurate records of patient interactions, care plans, and progress notes in compliance with organisational policies.
  • Schedule appointments, follow-ups, and necessary tests while ensuring that patients are informed of their care plans.
  • Assist clients in navigating healthcare systems, providing guidance on available resources and support services.
  • Monitor client outcomes and satisfaction levels, addressing any concerns or issues that may arise during the care process.
  • Collaborate with multidisciplinary teams to develop comprehensive care strategies tailored to individual patient needs.
  • Ensure compliance with relevant regulations and standards in all aspects of patient care coordination.

Skills and Experience

  • Proven experience in home & community care or assisted living environments with a focus on seniors
  • Demonstrate ability to manage multiple tasks simultaneously while maintaining attention to detail is crucial for the role
  • Knowledge of dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities
  • Certifications such as First Aid, CPR and medication administration are highly desirable
  • Strong observational skills to monitor patient condition and recognize early signs of health deterioration
  • Ability to perform heavy lifting safely using equipment when necessary
  • Excellent communication skills for effective collaboration with multidisciplinary teams and families
  • Familiarity with care planning software, patient monitoring tools, and documentation standards in healthcare settings
