Essential Requirements:
- Degree in
Occupational Therapy, Nursing, Social Work, Psychology
, or another relevant clinical field - Experience in
rehabilitation
and/or
complex care coordination - Strong communication
,
organisation
, and
problem-solving
skills - Confident working independently and collaboratively within a
multidisciplinary team - Compassionate, professional, and client-focused
- Understanding of healthcare regulations, protocols, and systems
- Experience supporting clients with catastrophic injuries
Your Role
As a Case Manager
at KICM, you'll play a vital role in coordinating and overseeing personalised rehabilitation and care packages tailored to each client's unique needs.
You will:
- Act as the
main point of contact
for clients, families, and professionals - Conduct
holistic assessments
to identify needs and improve quality of life - Develop, monitor, and adjust
care and rehabilitation plans
as clients progress - Liaise with
multidisciplinary teams
including OTs, Physiotherapists, SLTs, and support staff - Facilitate appointments, therapy sessions, and community-based services
- Prepare
comprehensive reports and documentation
for clients and stakeholders - Support clients and families in navigating
litigation, funding, and care planning - Recruit and manage support staff
in collaboration with the KICM HR team - Build positive relationships with
Solicitors and legal teams
involved in client care
