Essential Requirements:

  • Degree in
    Occupational Therapy, Nursing, Social Work, Psychology
    , or another relevant clinical field
  • Experience in
    rehabilitation
    and/or
    complex care coordination
  • Strong communication
    ,
    organisation
    , and
    problem-solving
    skills
  • Confident working independently and collaboratively within a
    multidisciplinary team
  • Compassionate, professional, and client-focused
  • Understanding of healthcare regulations, protocols, and systems
  • Experience supporting clients with catastrophic injuries

Your Role

As a Case Manager
at KICM, you'll play a vital role in coordinating and overseeing personalised rehabilitation and care packages tailored to each client's unique needs.

You will:

  • Act as the
    main point of contact
    for clients, families, and professionals
  • Conduct
    holistic assessments
    to identify needs and improve quality of life
  • Develop, monitor, and adjust
    care and rehabilitation plans
    as clients progress
  • Liaise with
    multidisciplinary teams
    including OTs, Physiotherapists, SLTs, and support staff
  • Facilitate appointments, therapy sessions, and community-based services
  • Prepare
    comprehensive reports and documentation
    for clients and stakeholders
  • Support clients and families in navigating
    litigation, funding, and care planning
  • Recruit and manage support staff
    in collaboration with the KICM HR team
  • Build positive relationships with
    Solicitors and legal teams
    involved in client care
