Are you passionate about supporting individuals with mental health needs? We are looking for a Care Navigator to join our dynamic and diverse community mental health team, to deliver high quality care.
The Bentley House Adult Community Mental Health Hub (CMHH) serves the diverse population of Harrow, who have secondary mental health needs. The post holder will have the opportunity to work in collaboration with a full multidisciplinary team including medical staff, nurses and allied health professionals. The CMHH is a trauma-informed service and operate an intervention-focused model.
The role is a fixed term contract for one year; an excellent opportunity to develop skills, knowledge and experience in community mental health. This is a full role from Monday - Friday; 9am - 5pm.
Bentley House is based in Harrow; a 5 minute walk from Harrow and Wealdstone station.
Please note that: This role requires successful candidates to be able to engage in Therapeutic Management of Violence and Aggression (TMVA) training, including the use of force techniques and the ability to attend resuscitation training and perform CPR. This is because of the high-acuity nature of the ward / community environment and the need to ensure the safety of both patients and staff. The role is conditional upon candidates being able to engage in TMVA training and resus training during the course of their employment whilst undertaking this role.
Main duties of the job
To provide referral management to and from Harrow Secondary Mental Health
Services including but not exclusive of signposting to voluntary sector resources.
To provide allocated patients with telephone and written signposting to services available in the community –based on detailed knowledge of the relevant access arrangements, eligibility criteria, service content of both statutory and voluntary sector services.
To develop a knowledge of services and information on local community services.
To maintain this knowledge through web search, initiative and other available resources.
To enable the impact of the service to be assessed and to inform the
improvement of other local services by keeping electronic records of all referrals/interventions and contributing to achievement of team Key Performance Indicators.

Main Responsibilities
To co-work with people who have mental health needs in a supportive, practical, empowering and holistic way towards better health.
To provide basic time limited psychosocial interventions to support service users in their recovery
To work as part of an integrated team including Consultant Psychiatrists, Social Workers, Nurses, Psychologists and Occupational Therapists in identifying and supporting people to reduce the risk of relapse.
To provide practical support with a range of issues including social isolation, benefits, housing, debt and accessing health and social care services.
To work on time limited interventions with people with a wide range of needs and have the ability to prioritise and case-manage those individuals.
To review and process referrals received from GP Practices, including those to be subsequently triaged and assessed by Community Mental Health Practitioners.
To actively telephone/contact patients with a view to assisting them through the process and making their onward appointments into community or secondary care service providers.
To maintain record of all activities carried out, i.e. outbound calls, calls received, archiving any received correspondence.
To actively answer telephone calls to CMHHs and deal with queries in a sensitive and timely way, liaising with clinical or managerial staff as appropriate.
To ensure messages/voicemails are dealt with appropriately and in a timely way.
To Carry Out Other Administrative Tasks As Required.

Experience
Essential criteria
- Mental Health background experience
- Some experience of working in health, social care or information and advice, in direct contact with service users - in a paid or voluntary capacity. Ideally within a Community Mental Health setting.
- Working with challenging or vulnerable patients who present with multiple or complex needs or risk history. Experience of person-centred planning

- Diploma/degree in a relevant field
Skills and Knowledge
Essential criteria
- Computer literate, able to use email, the internet and web searches as standard applications and databases
- Knowledge of Primary and Secondary care services and pathways
- Excellent communication skills, both verbal and written
- Numerate and literate, able to supply reports as required.
- Has, or can show ability to develop, a good knowledge of local services
- Multi-tasking utilising phone systems, IT & having excellent attention to detail
- Knowledge of personalisation and public health agendas, including holistic view of health and wellbeing
Desirable criteria
- Knowledge/experience of using SystmOne/electronic patient records system
