The role

As a Clinical Pharmacist in this practice, you will:

  • Lead medicines optimisation across the patient population
  • Carry out structured medication reviews, particularly for patients with long-term conditions or on complex medications
  • Ensure safe, effective, and cost-efficient prescribing
  • Provide consultations in person, by phone, or video for medicines queries
  • Contribute to QOF, prescribing incentive schemes, and safety audits
  • Work closely with community pharmacies, care homes, and the wider NHS

What we're looking for

  • GPhC registered pharmacist
  • Primary care experience preferred (hospital or community experience also considered)
  • Excellent communication skills with a patient-centred approach
  • Independent Prescribing qualification desirable
