Job Title: Social Worker (Hospital Discharge Team)
Location: Middlesbrough Metropolitan of Lewisham
Rate: £284 per day

An exciting job role has recently become available at the Middlesbrough Metropolitan of Lewisham. They are looking for a new member for their Hospital Discharge Team.

X4 workers required for a 12 month period
2 years post-qualifying experience required. Request to cover case work/reviews. This will involve the ability to undertake face-to-face visits 5 days a week with 2 days a week working from an office setting. Learning Disability Team – adults in transition and over 18 with a learning disability.

Requirements:

  • Assessing Needs: Conduct interviews and assessments to understand the needs of individuals and families.
  • Developing Care Plans: Create and implement care plans to support clients in achieving better outcomes.
  • Advocacy: Advocate for clients' rights and access to necessary services.
  • Collaboration: Work with other professionals in multidisciplinary teams to provide comprehensive support.

What we are looking for in a Social Worker:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
