Join our team at St Clements Surgery and help us deliver exceptional patient care.
We are looking for a motivated, organised and compassionate Care Co-ordinator to join our friendly and forward-thinking practice in the heart of Winchester. This is a key role supporting both our clinical and administrative teams, helping us maintain high-quality patient care and ensuring the smooth running of the surgery.
About the Role
As a Care Co-ordinator, you will provide a flexible mix of administrative and secretarial support across the practice. You'll work closely with GPs, nurses, and the wider administrative team, acting as a crucial link between patients, clinicians, and external healthcare agencies.
Your work will include day-to-day administrative tasks, document management, processing referrals, handling advice and guidance requests, managing non-NHS medical reports, supporting QOF activity, and coordinating chronic disease and drug monitoring invitations.
You will also support practice audits, research activities, medical student rotas, nursing home liaison, and other essential functions that contribute to efficient, safe, and high-quality care.
This is a varied and rewarding role suited to someone who is highly organised, proactive, and able to work both independently and as part of a team.
Key Responsibilities
Administrative
- Respond to patient and internal queries via NHS Mail and Accurx.
- Maintain accurate records and ensure the highest standards of confidentiality.
- Assist with safeguarding admin, recalls, long-term condition reviews and clinical searches.
- Support audits, reporting, and general practice administration.
Secretarial
- Process referrals through the NHS e-Referral System (ERS).
- Handle advice and guidance requests and two-week wait (2WW) referrals.
- Liaise with patients regarding referrals, appointments, and letters.
- Manage and process non-NHS medical reports (insurance, solicitor, DWP, capacity forms).
- Prepare and format letters and medical reports for GPs.
- Support clinicians with documentation and administrative tasks linked to their clinics.
Quality and QOF Support
- Ensure the practice meets QOF targets through accurate patient monitoring.
- Manage patient invitations for chronic disease reviews and drug monitoring.
- Support national health campaigns and research activities.
Collaborative Working
- Work closely with clinical and non-clinical colleagues to support smooth practice operations.
- Act as first point of contact for external health agencies and nursing homes.
- Contribute to continuous improvement and promote high-quality patient care.

- Prior experience in general practice or healthcare administration is an advantage but not essential
