The post holder will play a key role in the continued development of Primary Care Drug and Alcohol Services (PCDAS) in the RTE/Bridgend and M&C areas.

This will involve working primarily alongside GP's and the wider primary care team to provide a safe and effective service to service users with stable drug and alcohol misuse difficulties, as well as working alongside the core CDAT in the facilitation of transfer between CDAT and PCDAS services.

The post holder will need to be a registered nurse and have excellent clinical knowledge and experience of substance misuse.

Excellent communication, listening and organisational skills are essential and you will need to be able to work as a member of a team in a challenging and changeable environment, along with having excellent presentation, oral and written communication skills.

You will need to demonstrate sound clinical knowledge and awareness of current clinical practice in substance misuse and mental health, have excellent clinical assessment skills and have sound knowledge of safeguarding procedures.

The post holder will be expected to manage a caseload of patients within the Primary Care surgeries and work closely with the GP's They will also be involved in education and health promotion issues that are applicable to this patient group and work alongside other professionals who provide services to this patient group.

Establish rapport and maintain supportive professional relationships with service users and their carers, even when there are barriers to acceptance such as lack of insight or difficulties adjusting to complex life changes; recognising the stress associated with caring, offering and promoting carer's assessments as required.

Liaise with statutory and voluntary sector agencies in particular Children's Services, Social Services, Pharmacies and the Criminal Justice Agencies.

Provide specialist clinical reports as required e.g. Court, child protection case conferences and Probation.

Exercise personal and professional autonomy, make complex and critical judgments under the management of PCDAS Clinical Lead and have decision making skills to satisfy the expectations of the role.

To be responsible for providing a high quality of service within a defined area, and specifically to deliver on the following key areas.

Work as part of a multi-disciplinary, fostering good communication with colleagues of all disciplines.

Manage highly complex and contentious information effectively demonstrating excellent communication skills, both orally and in writing.

Ensure record keeping meets the standard required by the NMC, utilising paper-based or IT systems as directed by the Senior Nurse and ensuring information sharing protocols are understood and appropriately explained to service users

Be a registered nurse with significant and proven experience of working in substance misuse, preferably with experience of working within the area of Primary Care.

Possess clinical experience and specialist knowledge within the field of substance misuse and mental health.

Provide evidence of continuing professional development and post registration education.

Provide a skilful and comprehensive assessment of all patients ensuring risk assessment is an integral component of all clinical work.

Be responsible and accountable for applying appropriate interventions in negotiation with patients in order to promote quality of life, maximise independence and minimise the effects of addiction

Demonstrate good organisational and communication skills . In particular they should be able to confidently liaise with substance misuse partners including those in Primary Care and GP partners.

Demonstrate a good understanding of the legislative and policy framework underpinning the delivery of substance misuse services e.g. The All Wales Substance Misuse Strategy, the Welsh Assembly Government Substance Misuse Frameworks and appropriate NICE Guidelines.

Ensure all clinics are planned and delivered in a clinically effective and timely way in line with service needs.

To manage a clinical caseload.

Be responsible for ensuring policies, procedures and protocols are implemented and adhered to.

Ensure the environment is safe for patients, meets the required standard of cleanliness and all equipment is maintained in good working order and checked as necessary. Take a role in the transfer of stable cases where transition to PCDAS has been identified as appropriate including development of a care plan which is negotiated with the patient, ensuring that the agreed care is delivered and evaluated on a regular basis.

Manage and maintain a diary of all work activities, making it available for audit as necessary.

Have physical dexterity, skills and accuracy to administer oral/intramuscular medication and to monitor physical health using appropriate equipment e.g. thermometer, sphygmomanometer, alcometer.

Demonstrate ability to present cases to the multi-disciplinary team considering the range of care and treatment options available in line with the current evidence base and making suggestions/recommendations regarding the appropriate course of treatment.

Assess and manage critical/unpredictable situations that may arise in their area of work taking into account various risks.

Be responsible for ordering essential clinical supplies demonstrating an awareness of efficient resource management.

Where and when required supervise more junior and HCSW staff

Have keyboard skills and information technology skills including word processing, email, intranet and internet, power point for communication, correspondence, clinical reports.

Be able to perform regular light duties using some physical effort.

Advise and monitor the provision of nursing care and be responsible and accountable for ensuring care delivered is evidenced based and centred on the individual needs of the service user.

Participate in research and clinical multi-disciplinary audit, promoting a positive attitude towards the implementation of evidence-based practice within the team.
