We are looking for Clinical Pharmacists to join one of the primary care networks (PCNs) based in the North of Barnet working across 7 general practices. As a Clinical Pharmacist, you will have an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council. You will need to be self‑motivated and able to work on your own, have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews. Experience in General Practice would be an advantage but is not essential if the applicant is clinically up‑to‑date and understands the process of a full medication review in the primary care setting. Please note you must have UK registration or a right to work in the UK. We do not provide any form of sponsorship or work visas Main duties of the job This will be a patient facing role working across a number of general practices for one of the PCNs in Barnet, managing long‑term conditions; working directly with patients to assess and treat conditions, as well as promote self‑care. As a clinical pharmacist, you will work within your clinical competencies as part of a multi‑disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation as well as acute prescription requests, while addressing both the public health and social care needs of patients in the GP practice(s) that make up the PCN. About us Barnet Federated GPs CIC is a not for profit organisation, putting GPs and healthcare professionals at the forefront of Primary Healthcare provision in Barnet. The Federation is building a large clinical team who will work as one in developing Primary Care Services in Barnet and will support Primary Care Networks. The Federation was formed in 2015 and aims to improve the quality of health in Barnet through sustainable primary care. Its current membership consists of all 48 primary care practices within the borough. We do not provide any form of sponsorship or work visas Job responsibilities As a Clinical Pharmacist, you will be part of a multidisciplinary team to develop and run processes for repeat prescription reauthorisation, management of medicines on transfer of care and systems for safer prescribing. With a patient facing role in managing long‑term conditions; working directly with patients to assess and treat conditions, as well as promote self‑care. You will be part of a group of Primary Care Network clinical pharmacists working under the supervision; training and mentoring of a Senior Clinical Pharmacist. The main responsibilities include but are not limited to: Patient facing Care Home and Domiciliary Clinical Medication Reviews Management of common/minor/self‑limiting ailments Medicine information to practice staff and patients Telephone medicines support >Unplanned admissions Management of medicines at discharge from hospital Signposting Repeat prescribing The Job Description and Person Specification provides a full outline of responsibilities and requirement for this role. Person Specification Qualifications Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council. Minimum 2 years post‑qualifying. May hold or be working towards an independent prescribing qualification. Clinical Diploma. Experience An appreciation of the nature of GPs and general practices. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing. Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities. Excellent interpersonal, influencing and negotiating skills. Excellent written and verbal communication skills. Good knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports. Able to obtain and analyse complex technical information. Able to work under pressure and meet deadlines. Able to gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers. Work effectively, independently and as a team member. Able to identify and resolve risk management issues according to policy/protocol. Follows professional and organisational policies/procedures relation to performance management. In depth therapeutic and clinical knowledge and understanding of the principles of evidence‑based healthcare. Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long‑term conditions. Understand the principles of research governance.
