About the Role Specialist Occupational Therapist – Community Neuro & Stroke Team, Band 6, 30 hours per week, full time. Base: Ashton, Wigan & Leigh. Division: Community Services. We have an exciting opportunity for a Band 6 Specialist Occupational Therapist to join our well-established Community Neuro and Stroke Team, providing high-quality, evidence-based therapy to adults across Ashton, Wigan and Leigh. Key Responsibilities Provide a high standard of Occupational Therapy assessment and intervention for patients with neurological dysfunction in community and home settings. Take responsibility for a designated caseload, prioritising, planning and organising interventions effectively in line with clinical need. Work collaboratively with patients, carers and colleagues to establish meaningful rehabilitation goals using recognised goal-setting frameworks. Apply advanced clinical reasoning to formulate accurate diagnoses, prognoses and treatment plans for patients with complex or chronic presentations. Evaluate and modify treatment approaches to ensure interventions remain effective and evidence based. Contribute to discharge planning, risk assessment and long-term condition management. Provide advice, education, and support to patients, families and other professionals regarding neurological rehabilitation and self-management strategies. Supervise and support junior staff, therapy assistants and students on placement. Participate in service development, clinical audit, research and quality improvement projects. Ensure accurate and timely clinical documentation and adherence to professional and Trust standards. Promote and demonstrate compliance with professional codes of conduct, Trust policies and clinical governance frameworks. About You You will be a qualified Occupational Therapist with HCPC registration and experience working with adults who have neurological or complex physical conditions. You should demonstrate sound clinical reasoning, excellent communication and interpersonal skills, and the ability to work effectively within an interdisciplinary team. We are looking for someone who is proactive, flexible and compassionate, with a genuine commitment to providing high-quality, patient-centred care. Experience in community or stroke rehabilitation, and confidence in leading goal-based interventions will be advantageous. You will also demonstrate the ability to supervise others, contribute to the education of students, and engage in your own ongoing professional development through reflective practice and CPD activities.
