Applications are invited from highly motivated and experienced nurses with a demonstrable track record of practical application and theoretical knowledge of the care and treatment of people with complex mixtures of psychological, cognitive, functional, behavioural, physical and social risk usually related to mental disorder and frailty experienced in later life.The post holder will be expected to represent and lead on the OACMHT developmentof alternative pathways (holistic multi-agency and multidisciplinary) for people living with diagnosed/undiagnosed dementia in care homes within the locality, experiencing acute-frailty distress.The post holder will be responsible for the coordination and delivery of rapid response for urgent face to face assessment of those patients registered to the OA-CMHT.The post holder will lead on the provision of Dementia diagnostic and treatment clinics for complex older people living in care settings, to seek out and assess their mental health conditions, in order to inform the diagnostic process and to support subsequent interventions and treatment regimens and evaluation of that planned care.Main duties of the jobThe post holder will work to agreed job plan, which will include holding a small caseload of clients subject to CTP, undertake caseload management and running clinics primarily within the county care home sector or across the service area as the need arises.The post holder will undertake a full holistic assessment including the review of medicines, specifically Acetylcholinesterase Inhibitors and Anti-Psychotics for those diagnosed with Dementia within the locality care home sector; promote patient choice and effective outcomes.The post holder will be able to provide senior knowledge and experience within the team and take a senior position in the Multi-Disciplinary Team (MDT) to formulate & implement care plans and consider prescribing options.To accept clinical responsibility for a designated workload of specialised patients, and to organise this effectively. They will offer expert assessment of psychological, social problems and as an independent prescriber have an in-depth expert knowledge of the physical problems related to the medicines used in treatment.To undertake non-medical prescribing in line with legislation. However applicants who do not currently hold the qualification for the Band 7 role are encouraged to apply and, if successful, will need to attain the qualification within 24 months and will be appointed under the All Wales Annex 21 rules.This means they will be supported by a development plan. During the first year payment will be 70% of the top of Band 7 payscale and 2nd year will be 75% of the top of Band 7 payscale.Following successful completion of the Non-Medical Prescribing qualification (V300) the post holder will then be paid at band 7 (minimum point on the scale).Failure to achieve the qualification in the designated timeframe could result in re-deployment.As part of the Annex 21 process, there is no financial penalty to those who apply for this role where the percentage will cause a reduction in pay, subsequently a local protection would be put in place until the pay has caught up.You will be able to find a full job description and person specification attached within the supporting documents.The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.Interviews to be held on 22/12/2025The Health Board is committed to supporting its staff to fully embrace the need for bilingualism thereby enhancing patient and service user experiences.
