This post will include some early morning and evening work (Monday-Friday) on a rota basis (the team currently do a late shift 10am-6pm and an early shift 5:30am-12:30pm, both occur approximately once every 6 weeks). **Contract Type:** Permanent **Salary:** £25,110 - £32,090 per annumPlease note that we welcome/recommend a pre-interview visit, to give candidates the opportunity to understand the role but to also experience the environment and people they will be working with day-to-day.**Make a real difference in your community**Join **Waythrough**, one of the UK’s leading mental health and social support charities, and make a lasting impact every day.We support over **125,000 people each year** through our 200 services, empowering individuals to overcome challenges around mental health, housing, and substance use.**About the Role**An exciting opportunity has become available for position of Street Outreach Practitioner based within our Forward Leeds team.**What** **You’ll Do**Street Support Recovery Co-ordination is a diverse and rewarding role, key duties you will undertake are:* Manage a caseload, devising, implementing, and reviewing on-going recovery and treatment plans.* Facilitate and promote individuals’ progression from initial engagement into structured treatment.* Work in a proactive role to minimise and prevent rough sleeping and begging within Leeds by engaging and maintaining clients into treatment.* Reduce substance related harm to individuals and the wider community.* Working collaboratively with key partners and colleagues across the street support partnership to achieve the best health and wellbeing outcomes for individuals.* To support individuals to move into supported, more permanent accommodation.* To provide holistic support to adult service users and to coordinate their drug treatment, including assessment, recovery coordination, Psychosocial Interventions (PSI) delivery and multi-agency working.* As directed by the Lead Practitioner, the practitioner will be an integral member of the dynamic multi-agency team developing comprehensive and tailored individual interventions.**To Succeed in This Role, You’ll Need:*** Experience of holding a caseload.* Knowledge and/or experience of substance misuse.* Experience supporting vulnerable people, whose lives have been affected by alcohol and/or drug use.* A relevant qualification in Health and Social Care.To view the full job description and person specification, please **Please be advised, if you are viewing this role on Indeed, the link to the job description will not work.
