About the Role Set up and manage a range of sustainable Healthy Communities projects/programmes that deliver measurable impact for the community for a local contract area, focusing on tackling health inequalities. Complete any monitoring and evaluation requested by line manager in line with current data protection protocols. Deliver specific Healthy community project/programmes, such as Exercise Referral, gym‑based classes, and falls prevention, according to parameters discussed with line manager. About You Qualified instructor in a relevant discipline to deliver sessions independently (e.g., Gym Level 3, NGB Level 2 or higher). Experienced in planning, delivering, and monitoring sessions.
